Insurance cover for uk members

The following are the terms and conditions of the ISTA Group Insurance Policy for UK Members. They are valid for the period 1st April 2004 to 31st March 2005.

This policy is underwritten on a 'Claims Made' basis and therefore must be in force at the time a claim is made against you. If you have been previously insured then this policy will give retro-active cover for the treatments given during the previous period of cover.

TO ARRANGE COVER, FOLLOW THESE INSTRUCTIONS :

You must be a member of ISTA to qualify for this group insurance. Other therapies that you are qualified in can be covered, provided that they are listed below or the underwriter secures an addition for your specific skill.
Print off and complete the proposal form and include all documentation requested.
Enclose your cheque for the correct premium, make it payable to ËEMS Ltd.'
