Insurance cover for IRISH members

The following are the terms and conditions of the ISTA Group Insurance Policy for Irish Members.  They are valid for the period 1st April 2004 to 31st March 2005.

This policy is underwritten on a 'Claims Made' basis and therefore must be in force at the time a claim is made against you. If you have been previously insured then this policy will give retro-active cover for the treatments given during the previous period of cover.

TO ARRANGE COVER, FOLLOW THESE INSTRUCTIONS :

1. You must be a member of the Irish Scenar Practitioners Society to qualify for this group insurance. Other therapies that you are qualified in can be covered, provided that they are listed below or the underwriter secures an addition for your specific skill.

2. Obtain a copy of the insurance proposal form from your Society and include all documentation requested.

3. Enclose your cheque for the correct premium, make it payable to "EEMS Ltd."

4. Send all of the above to Education & Media Services Ltd. 15 St Lawrence Road South, Towcester, Northants, UK, NN12 6DR

ANNUAL PREMIUMS

Policies are issued on a 12 month basis commencing on the date of your Choice.

APPROVED THERAPIES : Cover in addition to S.C.E.N.A.R. Therapy.
